Variant summary: NBN c.772G>C (p.Glu258Gln) results in a conservative amino acid change located in the Nibrin, second BRCT domain of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251264 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.772G>C in individuals affected with Nijmegen Breakage Syndrome and no experimental evidence demonstrating its impact on protein function have been reported. Four submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

The p.E258Q variant (also known as c.772G>C), located in coding exon 7 of the NBN gene, results from a G to C substitution at nucleotide position 772. The glutamic acid at codon 258 is replaced by glutamine, an amino acid with highly similar properties. This alteration was identified in an individual diagnosed with breast cancer (Dutil J et al. Sci Rep, 2019 Nov;9:17769). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
