This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 172 of the TMEM67 protein (p.Arg172Gln). This variant is present in population databases (rs750950408, gnomAD 0.01%). This missense change has been observed in individual(s) with clinical features of TMEM67-related conditions (PMID: 19574260, 31019026, 34645491). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 217721). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The glutamine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic.

Variant summary: TMEM67 c.515G>A (p.Arg172Gln) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 2.4e-05 in 251282 control chromosomes. c.515G>A has been reported in the literature in individuals affected with Joubert Syndrome And Related Disorders (example: Doherty_2010, Bachmann-Gagescu_2015, Clark_2019, DeLaVega_2021). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 26092869, 19574260, 31738409, 31589614, 32000717, 31019026, 34645491, 30455918, 34964473, 34675960). ClinVar contains an entry for this variant (Variation ID: 217721). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic.
