FKTN (HGNC:3622): (fukutin) The protein encoded by this gene is a putative transmembrane protein that is localized to the cis-Golgi compartment, where it may be involved in the glycosylation of alpha-dystroglycan in skeletal muscle. The encoded protein is thought to be a glycosyltransferase and could play a role in brain development. Defects in this gene are a cause of Fukuyama-type congenital muscular dystrophy (FCMD), Walker-Warburg syndrome (WWS), limb-girdle muscular dystrophy type 2M (LGMD2M), and dilated cardiomyopathy type 1X (CMD1X). Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251212 control chromosomes. c.557A>G has been reported in the literature in the homozygous state in an individual affected with classic Walker-Warburg Syndrome (Manzini_2008). These data indicate that the variant may be associated with disease. At least one functional study found that the variant protein was retained in the ER as opposed to localizing to the golgi, but that it also retained the activity responsible for -DG glycosylation similar to that of the WT protein (Tachikawa_2012). The following publications have been ascertained in the context of this evaluation (PMID: 18752264, 22275357). ClinVar contains an entry for this variant (Variation ID: 554414). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. - |

This alteration results from a A to G substitution at nucleotide position 557, causing the histidine (H) at amino acid position 186 to be replaced by an arginine (R). This alteration was reported in the homozygous state in a patient diagnosed with classic Walker-Warburg syndrome (Manzini, 2008). Functional studies show that this alteration may affect folding and cellular localization of fukutin protein (Tachikawa, 2012). The p.H186R alteration is predicted to be deleterious by in silico analysis. Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. - |
