Genes affected
STXBP1 (HGNC:11444): (syntaxin binding protein 1) This gene encodes a syntaxin-binding protein. The encoded protein appears to play a role in release of neurotransmitters via regulation of syntaxin, a transmembrane attachment protein receptor. Mutations in this gene have been associated with infantile epileptic encephalopathy-4. Alternatively spliced transcript variants have been described. [provided by RefSeq, Feb 2010]

Pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsOct 17, 2022The c.875G>T (p.R292L) alteration is located in exon 10 (coding exon 10) of the STXBP1 gene. This alteration results from a G to T substitution at nucleotide position 875, causing the arginine (R) at amino acid position 292 to be replaced by a leucine (L). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This variant was reported de novo in multiple individuals with clinical features consistent with STXBP1-related developmental and epileptic encephalopathy (Stamberger, 2016; Zaganas, 2021). Two alterations at the same codon, c.875G>A (p.R292H) and c.874C>T (p.R292C), have also been reported de novo in individuals with clinical features consistent with STXBP1-related developmental and epileptic encephalopathy (Michaud, 2014; Stamberger, 2016; Uddin, 2017). This amino acid position is highly conserved in available vertebrate species. This missense alteration is located in a region that has a low rate of benign missense variation (Lek, 2016; Firth, 2009). This alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, this alteration is classified as pathogenic. -
