The P119L variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The P119L variant is not observed at a significant frequency in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The P119L variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. This substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. In summary, we interpret P119L to be a likely pathogenic variant. -

This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 119 of the COQ4 protein (p.Pro119Leu). This variant is present in population databases (rs773943371, gnomAD 0.02%). This missense change has been observed in individual(s) with coenzyme Q10 deficiency (PMID: 28540186). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 383650).
