Variant summary: BTK c.1559G>A (p.Arg520Gln) results in a conservative amino acid change located in the protein kinase domain (IPR000719) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 182482 control chromosomes (gnomAD) but has been reported in the literature in multiple individuals affected with X-linked Agammaglobulinemia. In some families it has also been reported to segregate with the disease (Hagemann_1994, Danielian_2003, Zhu_1994, Gaspar_1995, Fiorini_2004). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. A ClinVar submission (evaluation after 2014) cites the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 520 of the BTK protein (p.Arg520Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with X-linked agammaglobulinemia (XLA) (PMID: 7849721, 7880320, 11472359, 12217331, 12655572, 15661032, 18677443, 27980540). It has also been observed to segregate with disease in related individuals. This variant is also known as c.1691G>A. ClinVar contains an entry for this variant (Variation ID: 11378). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt BTK protein function with a positive predictive value of 95%.
