Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PM1PM2PM5PP3_StrongPP5_Very_Strong
The ENST00000328300.11(COL4A5):c.4931G>A(p.Cys1644Tyr) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. C1644R) has been classified as Uncertain significance.

not provided Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Dec 09, 2021 | For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects COL4A5 function (PMID: 20130921). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt COL4A5 protein function. ClinVar contains an entry for this variant (Variation ID: 24780). This missense change has been observed in individual(s) with Alport syndrome (PMID: 17277342). It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces cysteine, which is neutral and slightly polar, with tyrosine, which is neutral and polar, at codon 1638 of the COL4A5 protein (p.Cys1638Tyr). - |
X-linked Alport syndrome Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Victorian Clinical Genetics Services, Murdoch Childrens Research Institute | Sep 23, 2024 | Based on the classification scheme VCGS_Germline_v1.3.4, this variant is classified as Pathogenic. Following criteria are met: 0103 - Dominant negative and loss of function are known mechanisms of disease in this gene and are associated with X-linked Alport syndrome 1 (MIM#301050). Dominant negative is caused mostly by glycine substitutions that affect the conformation of the protein, and loss of function can be caused by either protein truncating or missense variants (PMIDs: 24046192, 12028435). (I) 0110 - This gene is associated with X-linked dominant disease. Males are typically more severely affected than females (PMID: 19965530). (I) 0115 - Variants in this gene are known to have variable expressivity. There is intrafamilial variability among affected carrier females, possibly due to variable X-inactivation (PMID: 14514738). (I) 0200 - Variant is predicted to result in a missense amino acid change from cysteine to tyrosine. (I) 0251 - This variant is heterozygous. (I) 0301 - Variant is absent from gnomAD (both v2 and v3). (SP) 0600 - Variant is located in the annotated C4 domain (DECIPHER). (I) 0703 - Other missense variants comparable to the one identified in this case have moderate previous evidence for pathogenicity. Changes to serine and tryptophan have been reported as pathogenic or likely pathogenic in individuals with Alport syndrome (ClinVar, LOVD, PMIDs: 26809805, 29270492). (SP). 0802 - This variant has moderate previous evidence of pathogenicity in unrelated individuals with Alport syndrome (ClinVar, LOVD, PMIDs: 17277342). Notably, this variant has been detected in patients with Alport syndrome with late onset renal failure without hearing loss or eye abnormalities (PMIDs: 17277342, VCGS cohort). (SP) 0901 - This variant has strong evidence for segregation with disease. It has been reported in a multi-generational family with Alport syndrome (PMID: 17277342). (SP) 1002 - This variant has moderate functional evidence supporting abnormal protein function. Functional analysis showed a significant reduction in heterotrimer formation and secretion (PMID: 20130921). (SP) Legend: (SP) - Supporting pathogenic, (I) - Information, (SB) - Supporting benign - |
