Genes affected
COL4A5 (HGNC:2207): (collagen type IV alpha 5 chain) This gene encodes one of the six subunits of type IV collagen, the major structural component of basement membranes. Mutations in this gene are associated with X-linked Alport syndrome, also known as hereditary nephritis. Like the other members of the type IV collagen gene family, this gene is organized in a head-to-head conformation with another type IV collagen gene so that each gene pair shares a common promoter. Alternatively spliced transcript variants have been identified for this gene. [provided by RefSeq, Aug 2010]

Classification made for transcript
Verdict is Likely_pathogenic. Variant got 6 ACMG points.
PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.01576044 fraction of the gene. Cryptic splice site detected, with MaxEntScore 4.6, offset of -35, new splice context is: aggGTaatc. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in inframe change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant X-108573655-G-A is Pathogenic according to our data. Variant chrX-108573655-G-A is described in ClinVar as [Pathogenic]. Clinvar id is 2584372.Status of the report is criteria_provided_single_submitter, 1 stars. Variant chrX-108573655-G-A is described in Lovd as [Pathogenic].

Likely pathogenic, criteria provided, single submitter | clinical testing | Ambry Genetics | Oct 18, 2024 | The c.546+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 9 of the COL4A5 gene. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; however, direct evidence is unavailable. The exact functional effect of the altered amino acid sequence is unknown; however, the impacted region is critical for protein function (Ambry internal data). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This variant was reported in individuals with features consistent with X-linked Alport syndrome (Knebelmann,1996). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. Based on the available evidence, this alteration is classified as likely pathogenic. - |
