The cytochrome P450 proteins are monooxygenases which catalyze many reactions involved in drug metabolism and synthesis of cholesterol, steroids and other lipids. This protein localizes to the mitochondrial inner membrane and is involved in the conversion of progesterone to cortisol in the adrenal cortex. Mutations in this gene cause congenital adrenal hyperplasia due to 11-beta-hydroxylase deficiency. Transcript variants encoding different isoforms have been noted for this gene. This sequence change replaces threonine, which is neutral and polar, with methionine, which is neutral and non-polar, at codon 318 of the CYP11B1 protein (p.Thr318Met). This variant is present in population databases (rs104894061, gnomAD 0.003%). This missense change has been observed in individuals with autosomal recessive adrenal hyperplasia (PMID: 8506298, 10487675, 28228528). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1173). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects CYP11B1 function (PMID: 8506298). For these reasons, this variant has been classified as Pathogenic. -
