Genes affected
PNP (HGNC:7892): (purine nucleoside phosphorylase) This gene encodes an enzyme which reversibly catalyzes the phosphorolysis of purine nucleosides. The enzyme is trimeric, containing three identical subunits. Mutations which result in nucleoside phosphorylase deficiency result in defective T-cell (cell-mediated) immunity but can also affect B-cell immunity and antibody responses. Neurologic disorders may also be apparent in patients with immune defects. A known polymorphism at aa position 51 that does not affect enzyme activity has been described. A pseudogene has been identified on chromosome 2. [provided by RefSeq, Jul 2008]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 12 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant located near the start codon (<100nt), not predicted to undergo nonsense mediated mRNA decay. There are 23 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 14-20472366-C-T is Pathogenic according to our data. Variant chr14-20472366-C-T is described in ClinVar as [Pathogenic]. Clinvar id is 13995.Status of the report is criteria_provided_single_submitter, 1 stars.

Purine-nucleoside phosphorylase deficiency Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testingInvitaeJan 15, 2023For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 13995). This premature translational stop signal has been observed in individual(s) with purine nucleoside phosphorylase deficiency (PMID: 9737781). This variant is present in population databases (rs104894455, gnomAD 0.003%). This sequence change creates a premature translational stop signal (p.Arg24*) in the PNP gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in PNP are known to be pathogenic (PMID: 24767876). -
