Vitamin B6 is a required co-factor for enzymes involved in both homocysteine metabolism and synthesis of neurotransmitters such as catecholamine. Mutations in this gene result in pyridoxamine 5'-phosphate oxidase (PNPO) deficiency, a form of neonatal epileptic encephalopathy. [provided by RefSeq, Oct 2008]

In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 229 of the PNPO protein (p.Arg229Trp). This variant is present in population databases (rs104894629, gnomAD 0.004%). This missense change has been observed in individual(s) with epileptic encephalopathy (PMID: 15772097). ClinVar contains an entry for this variant (Variation ID: 6523). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PNPO protein function. Experimental studies have shown that this missense change affects PNPO function (PMID: 15772097, 19759001, 24645144, 27419045). This variant disrupts the p.Arg229 amino acid residue in PNPO. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 23708187, 24266778, 28133863, 28985901). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. -
