This sequence change replaces arginine, which is basic and polar, with proline, which is neutral and non-polar, at codon 562 of the BTK protein (p.Arg562Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with agammaglobulinemia (PMID: 7809124, 10678660). ClinVar contains an entry for this variant (Variation ID: 11394). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt BTK protein function with a positive predictive value of 80%. This variant disrupts the p.Arg562 amino acid residue in BTK. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 7880320, 11742281, 16951917, 17765309, 19904586). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The R562P missense variant in the BTK gene has been reported previously in association withX-linked agammaglobulinemia (Vihinen et al., 1994; Curtis et al., 2000). This pathogenic variant occurs in the protein kinasedomain of the BTK protein near other residues with reported pathogenic variants (Vihinen et al.,1994). R562P impairs the kinase activity of the resulting protein despite a lack of effect on proteinlevels (Maniar et al., 1995). Additionally, the R562P variant was not observed in approximately6,500 individuals of European and African American ancestry in the NHLBI Exome SequencingProject, indicating it is not a common benign variant in these populations. Therefore, we interpret R562P as a pathogenic variant. -
