MVK (HGNC:7530): (mevalonate kinase) This gene encodes the peroxisomal enzyme mevalonate kinase. Mevalonate is a key intermediate, and mevalonate kinase a key early enzyme, in isoprenoid and sterol synthesis. Mevalonate kinase deficiency caused by mutation of this gene results in mevalonic aciduria, a disease characterized psychomotor retardation, failure to thrive, hepatosplenomegaly, anemia and recurrent febrile crises. Defects in this gene also cause hyperimmunoglobulinaemia D and periodic fever syndrome, a disorder characterized by recurrent episodes of fever associated with lymphadenopathy, arthralgia, gastrointestinal dismay and skin rash. This missense change has been observed in individual(s) with hyper IgD syndrome, mevalonate kinase deficiency, and/or recurrent fever of unknown origin (PMID: 11313768, 15536479, 21708801). Variant summary: MVK c.442G>A (p.Ala148Thr) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 251282 control chromosomes. c.442G>A has been observed in compound heterozygous and homozygous individuals affected with Hyperimmunoglobulin D with periodic fever (Houten_2001, D'Osualdo_2005, Bader-Meunier_2011). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 21708801, 15536479, 11313768). ClinVar contains an entry for this variant (Variation ID: 97592).
