Genes affected
GLI3 (HGNC:4319): (GLI family zinc finger 3) This gene encodes a protein which belongs to the C2H2-type zinc finger proteins subclass of the Gli family. They are characterized as DNA-binding transcription factors and are mediators of Sonic hedgehog (Shh) signaling. The protein encoded by this gene localizes in the cytoplasm and activates patched Drosophila homolog (PTCH) gene expression. It is also thought to play a role during embryogenesis. Mutations in this gene have been associated with several diseases, including Greig cephalopolysyndactyly syndrome, Pallister-Hall syndrome, preaxial polydactyly type IV, and postaxial polydactyly types A1 and B. ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 14 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 10 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 7-41964641-C-CA is Pathogenic according to our data. Variant chr7-41964641-C-CA is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 376814.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

This sequence change creates a premature translational stop signal (p.Glu1478*) in the GLI3 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 103 amino acid(s) of the GLI3 protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with clinical features of GLI3-related conditions and/or Greig cephalopolysyndactyly syndrome (PMID: 20672375, 24736735, 26508445, 31399769). ClinVar contains an entry for this variant (Variation ID: 376814). This variant disrupts a region of the GLI3 protein in which other variant(s) (p. Thr1488Lysfs*23) have been determined to be pathogenic (PMID: 24736735). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -
