Genes affected
TSC2 (HGNC:12363): (TSC complex subunit 2) This gene is a tumor suppressor gene that encodes the growth inhibitory protein tuberin. Tuberin interacts with hamartin to form the TSC protein complex which functions in the control of cell growth. This TSC protein complex negatively regulates mammalian target of rapamycin complex 1 (mTORC1) signaling which is a major regulator of anabolic cell growth. Mutations in this gene have been associated with tuberous sclerosis and lymphangioleiomyomatosis. [provided by RefSeq, May 2022]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 12 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.013827434 fraction of the gene. Cryptic splice site detected, with MaxEntScore 3.3, offset of 48, new splice context is: cggGTgaat. Cryptic site results in inframe change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 16-2085027-G-A is Pathogenic according to our data. Variant chr16-2085027-G-A is described in ClinVar as [Likely_pathogenic]. Clinvar id is 448732.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Tuberous sclerosis 2 Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testingVictorian Clinical Genetics Services, Murdoch Childrens Research InstituteOct 10, 2024Based on the classification scheme VCGS_Germline_v1.3.5, this variant is classified as Pathogenic. Following criteria are met: 0102 - Loss of function is a known mechanism of disease in this gene and is associated with tuberous sclerosis-2 (MIM#613254). (I) 0107 - This gene is associated with autosomal dominant disease. (I) 0115 - Variants in this gene are known to have variable expressivity. Clinical manifestations demonstrate great phenotypic variability, where even within families the clinical symptoms can vary significantly among individuals (PMID: 31018109). (I) 0211 - Canonical splice site variant without proven consequence on splicing (no functional evidence available). (SP) 0251 - This variant is heterozygous. (I) 0301 - Variant is absent from gnomAD (v2, v3 and v4). (SP) 0505 - Abnormal splicing is predicted by in silico tools and affected nucleotide is highly conserved. (SP) 0701 - Other splice variants comparable to the one identified in this case have very strong previous evidence for pathogenicity. c.4569+1G>T has been reported once as likely pathogenic (ClinVar). c.4569+2T>C has been reported in one individual with tuberous sclerosis and classified as pathogenic (PMID: 29476190) and has been submitted to ClinVar twice as likely pathogenic. c.4569+1G>C has been reported once as pathogenic in an individual with tuberous sclerosis (PMID: 31525612). c.4569+2T>A and c.4569+2T>G have also each been reported once as likely pathogenic (ClinVar). (SP) 0801 - This variant has strong previous evidence of pathogenicity in unrelated individuals. This variant has been reported as likley pathogenic twice and pathogenic twice (ClinVar) and in an individual fetus diagnosed with tuberous sclerosis (PMID: 38806662). (SP) 1007 - No published functional evidence has been identified for this variant. (I) 1203 - This variant has been shown to be de novo in the proband (parental status confirmed) (by trio analysis). (SP) Legend: (SP) - Supporting pathogenic, (I) - Information, (SB) - Supporting benign -
Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpOct 03, 2023This sequence change affects a donor splice site in intron 35 of the TSC2 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in TSC2 are known to be pathogenic (PMID: 10205261, 17304050). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individual(s) with tuberous sclerosis complex (PMID: 29476190, 31525612). ClinVar contains an entry for this variant (Variation ID: 448732). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic. -
