MUTYH (HGNC:7527): (mutY DNA glycosylase) This gene encodes a DNA glycosylase involved in oxidative DNA damage repair. The enzyme excises adenine bases from the DNA backbone at sites where adenine is inappropriately paired with guanine, cytosine, or 8-oxo-7,8-dihydroguanine, a major oxidatively damaged DNA lesion. The protein is localized to the nucleus and mitochondria. This gene product is thought to play a role in signaling apoptosis by the introduction of single-strand breaks following oxidative damage. Mutations in this gene result in heritable predisposition to colorectal cancer, termed MUTYH-associated polyposis (MAP). This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 359 of the MUTYH protein (p.Pro359Leu). This variant is present in population databases (no rsID available, gnomAD 0.005%). This missense change has been observed in individual(s) with clinical features of MUTYH-associated polyposis (PMID: 17949294, 20618354; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is also known as c.1034C>T (p.Pro345Leu). ClinVar contains an entry for this variant (Variation ID: 406827). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. The p.P359L variant (also known as c.1076C>T), located in coding exon 12 of the MUTYH gene, results from a C to T substitution at nucleotide position 1076. The proline at codon 359 is replaced by leucine, an amino acid with similar properties. This alteration has been detected in a patient with more than 5 polyps or colorectal cancer and in a second patient with approximately 20 adenomas in conjunction with MUTYH p.G396D (phase unknown) (Olschwang S et al. Genet Test. 2007 Fall;11(3):315-20; Morak M et al. Clin Genet. 2010 Oct;78(4):353-63). This alteration was also identified in an individual with colon polyposis in conjunction with MUTYH p.Y179C (phase unknown) (Ambry internal data). Of note, this alteration is also known as c.1034C>T (p.P345L) in published literature. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

This variant is denoted MUTYH c.1076C>T at the cDNA level, p.Pro359Leu (P359L) at the protein level, and results in the change of a Proline to a Leucine (CCC>CTC). This variant has been reported with the common pathogenic variant Gly396Asp in an individual with colorectal adenomas, although phase was not proven (Morak 2010). MUTYH Pro359Leu was not observed at a significant allele frequency in large population cohorts (Lek 2016). This variant is not located in a known functional domain. In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect. Based on currently available evidence, it is unclear whether MUTYH Pro359Leu is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. Of note, MUTYH-Associated Polyposis (MAP) is a recessive condition associated with two MUTYH pathogenic variants on opposite chromosomes. -
