This variant is present in population databases (no rsID available, gnomAD 0.0009%). This missense change has been observed in individuals with hypertrophic cardiomyopathy (PMID: 9541100, 9541104, 18803133; Invitae). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 407308). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects MYBPC3 function (PMID: 12386147, 12787675). For these reasons, this variant has been classified as Pathogenic. -

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsNov 17, 2020The p.N755K variant (also known as c.2265C>A), located in coding exon 23 of the MYBPC3 gene, results from a C to A substitution at nucleotide position 2265. The asparagine at codon 755 is replaced by lysine, an amino acid with similar properties. This variant was detected in multiple relatives with hypertrophic cardiomyopathy (HCM) from a single family, and based on haplotype was determined to have occurred as de novo in the father of the presenting proband (Yu B et al. J Med Genet, 1998 Mar;35:183-8; Yu B et al. J Med Genet, 1998 Mar;35:205-10). This variant was also reported in another HCM case and in one individual from an HCM testing cohort, although clinical details were limited (Rudziski T et al. Kardiol Pol, 2008 Aug;66:821-5; discussion 826-7; Walsh R et al. Genet Med, 2017 02;19:192-203). One study suggested this alteration may affect protein function; however, the clinical impact of these findings in not known (Moolman-Smook J et al. Circ Res, 2002 Oct;91:704-11). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
