Variant summary
Our verdict is Pathogenic. Variant got 12 ACMG points: 12P and 0B. PM2PP3_ModeratePP5_Very_Strong
The NM_004006.3(DMD):c.3603+3A>T variant causes a splice region, intron change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Pathogenic (★★).

Duchenne muscular dystrophy Pathogenic:2
This sequence change falls in intron 26 of the DMD gene. It does not directly change the encoded amino acid sequence of the DMD protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). This variant has been observed in individuals with Becker muscular dystrophy (BMD) (PMID: 16770791, 19230662, 21515508). ClinVar contains an entry for this variant (Variation ID: 409882). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in activation of a cryptic splice site and introduces a premature termination codon (PMID: 19230662). The resulting mRNA is expected to undergo nonsense-mediated decay. For these reasons, this variant has been classified as Pathogenic. -

Published functional studies, including RNA sequencing, showed that c.3603+3A>T resulted in altered splicing, leading to the production of a truncated transcript and significantly reduced expression of normal transcript; immunostaining of muscle biopsies showed faint and patchy normal protein expression, consistent with RNA sequencing (Baskin et al., 2009; Okubo et al., 2017); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports a deleterious effect on splicing; This variant is associated with the following publications: (PMID: 25525159, 9536098, 21515508, 17576681, 29196072, Garca-Acero2018[casereport], 27593222, 19230662, 28859693, 31397097, 16770791, 22294058) -
