Variant summary
Our verdict is Uncertain significance. The variant received 5 ACMG points: 5P and 0B. PM1PM2PM4_Supporting
The NM_024675.4(PALB2):c.3264_3266dupTGT(p.Val1089dup) variant causes a disruptive inframe insertion change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.000000684 in 1,461,864 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. V1089V) has been classified as Benign.

Hereditary cancer-predisposing syndrome Uncertain:2
The c.3264_3266dupTGT variant (also known as p.V1089dup), located in coding exon 12 of the PALB2 gene, results from an in-frame duplication of TGT at nucleotide positions 3264 to 3266. This results in the duplication of an extra valine residue between codons 1089 and 1090. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
This variant causes the duplication of a single amino acid from the BRCA2/ RAD51 binding domain of the PALB2 protein. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in a few individuals affected with breast cancer (PMID: 26564480; Color internal data). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Familial cancer of breast Uncertain:1
This variant, c.3264_3266dup, results in the insertion of 1 amino acid(s) of the PALB2 protein (p.Val1089dup), but otherwise preserves the integrity of the reading frame. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with PALB2-related conditions. ClinVar contains an entry for this variant (Variation ID: 410208). Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
