Variant summary
Our verdict is Pathogenic. The variant received 17 ACMG points: 17P and 0B. PM1PM2PP2PP3_StrongPP5_Very_Strong
The NM_000350.3(ABCA4):c.5932A>G(p.Lys1978Glu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.000000684 in 1,461,794 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 13/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★).

This sequence change replaces lysine, which is basic and polar, with glutamic acid, which is acidic and polar, at codon 1978 of the ABCA4 protein (p.Lys1978Glu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with ABCA4-related conditions (PMID: 25066811, 28041643; internal data). ClinVar contains an entry for this variant (Variation ID: 417994). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ABCA4 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -
The K1978E variant has been published as a pathogenic variant in association with Stargardt disease (Zernant et al., 2014) and observed in trans with other pathogenic variants in patients tested at GeneDx. The K1978E variant is not observed in large population cohorts (Lek et al., 2016). The K1978E variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. Missense variants in nearby residues (G1977S, T1979I, T1981R) have been reported in the Human Gene Mutation Database in association with ABCA4-related disorders (Stenson et al., 2014). In summary, based on the currently available information, this variant is likely pathogenic. -

Severe early-childhood-onset retinal dystrophy Pathogenic:1
The variant is not observed in the gnomAD v2.1.1 dataset. In silico tool predictions suggest damaging effect of the variant on gene or gene product (REVEL: 0.99; 3Cnet: 0.99). Same nucleotide change resulting in same amino acid change has been previously reported to be associated with ABCA4 related disorder (ClinVar ID: VCV000417994 / PMID: 25066811). The variant has been reported to be in trans with a pathogenic variant as either compound heterozygous or homozygous in at least one similarly affected unrelated individual (PMID: 25066811). Therefore, this variant is classified as likely pathogenic according to the recommendation of ACMG/AMP guideline. -
