This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. In a study of patients with a personal and/or family histories that were concerning for Hereditary Diffuse Gastric Cancer syndrome, this variant was been observed in a patient with breast cancer at age 59, who also had a family history of pancreatic and other cancers (Hansford S et al. JAMA Oncol, 2015 Apr;1:23-32). This variant was also reported in a patient diagnosed with pancreatic cancer under the age of fifty (Hutchings D et al. Mod Pathol, 2019 12;32:1806-1813). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

The normal sequence, with the bases that are deleted in brackets, is AAGA[delTACA]GGCC. The deletion causes a frameshift which changes a Tyrosine to a Glycine at codon 2791, and creates a premature stop codon at position 14 of the new reading frame. This variant is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. ATM c.8371_8374delTACA, also published as ATM c.8369GATAC>G using alternate nomenclature, has been observed in one individual with breast cancer and a strong family history of gastric cancer as well as in one individual with a Lynch syndrome-associated cancer and/or polyps (Hansford 2015, Yurgelun 2015). Based on the currently available information, we consider this deletion to be a likely pathogenic variant. -
