Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_003482.4(KMT2D):c.1143del(p.Thr382LeufsTer20) variant causes a frameshift change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. Variant has been reported in ClinVar as Likely pathogenic (★★). Variant results in nonsense mediated mRNA decay.

Genes affected
KMT2D (HGNC:7133): (lysine methyltransferase 2D) The protein encoded by this gene is a histone methyltransferase that methylates the Lys-4 position of histone H3. The encoded protein is part of a large protein complex called ASCOM, which has been shown to be a transcriptional regulator of the beta-globin and estrogen receptor genes. Mutations in this gene have been shown to be a cause of Kabuki syndrome. [provided by RefSeq, Oct 2010]

Pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Dec 21, 2016 | The c.1143delC variant in the KMT2D gene has not been reported previously as a pathogenic variant nor as a benignvariant, to our knowledge. The c.1143delC variant causes a frameshift starting with codon Threonine 382, changesthis amino acid to a Leucine residue, and creates a premature Stop codon at position 20 of the new reading frame, denoted p.Thr382LeufsX20. This variant is predicted to cause loss of normal protein function either through proteintruncation or nonsense-mediated mRNA decay. The c.1143delC variant was not observed in approximately 6100individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is nota common benign variant in these populations. We interpret c.1143delC as a pathogenic variant. - |
