The p.K2673R variant (also known as c.8018A>G), located in coding exon 17 of the BRCA2 gene, results from an A to G substitution at nucleotide position 8018. The lysine at codon 2673 is replaced by arginine, an amino acid with highly similar properties. This amino acid position is conserved. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
This missense variant replaces lysine with arginine at codon 2673 of the BRCA2 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with breast cancer (PMID: 37664050). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

This variant is denoted BRCA2 c.8018A>G at the cDNA level, p.Lys2673Arg (K2673R) at the protein level, and results in the change of a Lysine to an Arginine (AAA>AGA). Using alternate nomenclature, this variant would be defined as BRCA2 8246A>G. This variant has not, to our knowledge, been published in the literature as pathogenic or benign. BRCA2 Lys2673Arg was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Lysine and Arginine share similar properties, this is considered a conservative amino acid substitution. BRCA2 Lys2673Arg occurs at a position where amino acids with properties similar to Lysine are tolerated across species and is located in the DNA binding domain . In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether BRCA2 Lys2673Arg is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
