ACVRL1 (HGNC:175): (activin A receptor like type 1) This gene encodes a type I cell-surface receptor for the TGF-beta superfamily of ligands. It shares with other type I receptors a high degree of similarity in serine-threonine kinase subdomains, a glycine- and serine-rich region (called the GS domain) preceding the kinase domain, and a short C-terminal tail. The encoded protein, sometimes termed ALK1, shares similar domain structures with other closely related ALK or activin receptor-like kinase proteins that form a subfamily of receptor serine/threonine kinases. Mutations in this gene are associated with hemorrhagic telangiectasia type 2, also known as Rendu-Osler-Weber syndrome 2. [provided by RefSeq, Jul 2008]

Therefore, this variant has been classified as Likely Pathogenic. This sequence change replaces proline with threonine at codon 424 of the ACVRL1 protein (p.Pro424Thr). The proline residue is highly conserved and there is a small physicochemical difference between proline and threonine. This variant is not present in population databases (ExAC no frequency). This variant has been observed in individuals affected with hereditary hemorrhagic telangiectasia (PMID: 9245985) and pulmonary arterial hypertension (PMID: 18159113, Invitae). ClinVar contains an entry for this variant (Variation ID: 426028). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. Variants that disrupt the p.Pro424 amino acid residue in ACVRL1 have been observed in affected individuals (PMID: 9245985, 18159113, 15712271, 22781769). This suggests that it is a clinically significant residue, and that other variants that disrupt this residue are likely to be causative of disease. -
