Variant summary: DSP c.2552T>A (p.Leu851Gln) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 5.6e-05 in 251432 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in DSP causing Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy (5.6e-05 vs 0.0002), allowing no conclusion about variant significance. c.2552T>A has been reported in the literature in individuals affected with sudden cardiac death (Bagnall_2016). These report(s) do not provide unequivocal conclusions about association of the variant with Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 27332903). The p.L851Q variant (also known as c.2552T>A), located in coding exon 18 of the DSP gene, results from a T to A substitution at nucleotide position 2552. The leucine at codon 851 is replaced by glutamine, an amino acid with dissimilar properties. This variant was identified in one individual with sudden unexplained cardiac death (Bagnall RD et al. N. Engl. J. Med., 2016 Jun;374:2441-52). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
