Variant summary

Our verdict is Pathogenic. Variant got 11 ACMG points: 11P and 0B. PM1PM2PM5PP2PP3_Strong

The NM_001370259.2(MEN1):c.1127T>G(p.Leu376Arg) variant causes a missense change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. L376M) has been classified as Uncertain significance.

Genes affected
MEN1 (HGNC:7010): (menin 1) This gene encodes menin, a tumor suppressor associated with a syndrome known as multiple endocrine neoplasia type 1. Menin is a scaffold protein that functions in histone modification and epigenetic gene regulation. It is thought to regulate several pathways and processes by altering chromatin structure through the modification of histones. [provided by RefSeq, May 2019]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 11 ACMG points.

PM1
In a hotspot region, there are 2 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 2 benign, 11 uncertain in NM_001370259.2
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr11-64805693-A-G is described in ClinVar as [Likely_pathogenic]. Clinvar id is 428014.Status of the report is criteria_provided_single_submitter, 1 stars.
PP2
Missense variant where missense usually causes diseases, MEN1
PP3
MetaRNN computational evidence supports a deleterious effect, 0.96

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsMar 22, 2016The p.L376R variant (also known as c.1127T>G), located in coding exon 7 of the MEN1 gene, results from a T to G substitution at nucleotide position 1127. The leucine at codon 376 is replaced by arginine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. An alteration at this same amino acid position (p.L376P) was reported in a study of 200 unrelated Swedish index cases in a patient diagnosed with primary hyperparathyroidism, endocrine enterpancreatic tumor, and an anterior pituitary tumor. (Tham et al. J of Clinical Endocrinology & Metab. 2007. 92(9): 3389-3395). This amino acid position is completely conserved on sequence alignment. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear. -
