The p.L263P variant (also known as c.788T>C), located in coding exon 4 of the MEN1 gene, results from a T to C substitution at nucleotide position 788. The leucine at codon 263 is replaced by proline, an amino acid with similar properties. This alteration has been detected in at least one individual with a clinical diagnosis of multiple endocrine neoplasia type 1 (Ambry internal data). Based on internal structural analysis, this variant is predicted to result in a significant decrease in structural stability (Huang J et al. Nature. 2012 Feb;482:542-6; Xu S et al. Angew Chem Int Ed Engl. 2018 02;57:1601-1605). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with MEN1-related disease. ClinVar contains an entry for this variant (Variation ID: 428042). This variant is not present in population databases (ExAC no frequency). This sequence change replaces leucine with proline at codon 263 of the MEN1 protein (p.Leu263Pro). The leucine residue is highly conserved and there is a moderate physicochemical difference between leucine and proline. -
