Variant summary

Our verdict is Pathogenic. Variant got 13 ACMG points: 13P and 0B. PM1PM2PM5PP2PP3_StrongPP5_Moderate

The NM_001370259.2(MEN1):c.1258A>T(p.Ile420Phe) variant causes a missense change. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. I420S) has been classified as Likely pathogenic.

Genes affected
MEN1 (HGNC:7010): (menin 1) This gene encodes menin, a tumor suppressor associated with a syndrome known as multiple endocrine neoplasia type 1. Menin is a scaffold protein that functions in histone modification and epigenetic gene regulation. It is thought to regulate several pathways and processes by altering chromatin structure through the modification of histones. [provided by RefSeq, May 2019]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 13 ACMG points.

PM1
In a hotspot region, there are 6 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 10 uncertain in NM_001370259.2
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr11-64805125-A-C is described in ClinVar as [Likely_pathogenic]. Clinvar id is 428085.Status of the report is criteria_provided_single_submitter, 1 stars.
PP2
Missense variant in gene, where missense usually causes diseases (based on misZ statistic), MEN1. . Trascript score misZ 4.1921 (greater than threshold 3.09). GenCC has associacion of gene with multiple endocrine neoplasia type 1, pituitary gigantism, familial isolated hyperparathyroidism.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.953
PP5
Variant 11-64805126-T-A is Pathogenic according to our data. Variant chr11-64805126-T-A is described in ClinVar as [Likely_pathogenic]. Clinvar id is 428069.Status of the report is criteria_provided_single_submitter, 1 stars.

Likely pathogenic, criteria provided, single submitterclinical testingAmbry GeneticsOct 06, 2015The p.I420F variant (also known as c.1258A>T), located in coding exon 8 of the MEN1 gene, results from an A to T substitution at nucleotide position 1258. The isoleucine at codon 420 is replaced by phenylalanine, an amino acid with highly similar properties. A different alteration at the same codon, p.I420N, was detected in a proband with a clinical diagnosis of MEN1 and an intracranial ependymoma. Neither healthy parent carried this alteration, so it was presumed to be de novo (Al-Salameh, A et al. J Endocrinol Invest. 2010 May;33(5):353-6). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6498 samples (12996 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.05% (greater than 1,900 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. This alteration is predicted to be probably damaging and deleterious by PolyPhen and SIFT in silico analyses, respectively. In addition, based on Ambry internal structural analysis, this variant is anticipated to result in a significant decrease in structural stability (Huang, J et al. Nature. 2012 Feb 12;482(7386):542-6). Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
