Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MSH6 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. ClinVar contains an entry for this variant (Variation ID: 428381). This sequence change replaces glycine, which is neutral and non-polar, with glutamic acid, which is acidic and polar, at codon 430 of the MSH6 protein (p.Gly430Glu). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with MSH6-related conditions. -

The p.G430E variant (also known as c.1289G>A), located in coding exon 4 of the MSH6 gene, results from a G to A substitution at nucleotide position 1289. The glycine at codon 430 is replaced by glutamic acid, an amino acid with similar properties. One study found that codon 430, which occurs in the mismatch binding domain of the MSH6 protein, forms a nonspecific interaction with one nucleotide of a mismatched pair during simulations, however the authors did not study the effect of amino acid substitutions on this interaction (Sharma, M et al. Biophys J. 2014 Jun 3;106(11):2483-92). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 65,000 alleles tested) in our clinical cohort. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be probably damaging and deleterious by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of p.G430E remains unclear. -
