Disruption of this splice site has been observed in individuals with Birt-Hogg-Dubé syndrome (PMID: 15852235, 29720200). This variant is not present in population databases (gnomAD no frequency). This sequence change affects a donor splice site in intron 9 of the FLCN gene. RNA analysis indicates that disruption of this splice site induces altered splicing and may result in an absent or disrupted protein product. Studies have shown that disruption of this splice site results in retention of part of intron 9 sequence and introduces a premature termination codon (PMID: 29720200). The resulting mRNA is expected to undergo nonsense-mediated decay. For these reasons, this variant has been classified as Pathogenic. -

Also designated as IVS9+1G>A in published literature, this alteration was seen in a family with nine members affected with Birt-Hogg-Dube syndrome. Clinical findings included fibrofolliculomas, renal tumors, with lung cysts, and pneumothoraces (Schmidt LS et al. Am. J. Hum. Genet. 2005 Jun; 76(6):1023-33). This alteration was also identified in a Japanese male with multiple lung cysts and a personal and family history of pneumothorax whose RNA showed out of frame inclusion of 130 nucleotides of intron 6 (referred to as intron 9) (Furuya M et al. BMC Med Genomics, 2018 May;11:42). In addition to the clinical data presented in the literature, since alterations that disrupt the canonical splice donor site are typically deleterious in nature, this alteration is interpreted as a disease-causing mutation. -
