PTCH1 (HGNC:9585): (patched 1) This gene encodes a member of the patched family of proteins and a component of the hedgehog signaling pathway. Hedgehog signaling is important in embryonic development and tumorigenesis. The encoded protein is the receptor for the secreted hedgehog ligands, which include sonic hedgehog, indian hedgehog and desert hedgehog. Following binding by one of the hedgehog ligands, the encoded protein is trafficked away from the primary cilium, relieving inhibition of the G-protein-coupled receptor smoothened, which results in activation of downstream signaling. Mutations of this gene have been associated with basal cell nevus syndrome and holoprosencephaly. [provided by RefSeq, Aug 2017]

Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PTCH1 protein function. This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 1167 of the PTCH1 protein (p.Gly1167Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with basal cell nevus syndrome (PMID: 10890722, 15712338, 18502968; Invitae). ClinVar contains an entry for this variant (Variation ID: 428838). -

The p.G1167R variant (also known as c.3499G>A), located in coding exon 21 of the PTCH1 gene, results from a G to A substitution at nucleotide position 3499. The glycine at codon 1167 is replaced by arginine, an amino acid with dissimilar properties. This alteration has been detected in multiple patients meeting clinical diagnostic criteria for nevoid basal cell carcinoma syndrome (NBCCS) (Barreto, DC et al. J Dent Res. 2000 Jun;79(6):1418-22; Pastorino, L et al. Hum Mutat. 2005 Mar;25(3):322-3). In one study, this alteration was detected in a 42-year-old male with NBCCS who had a history of multiple jaw keratocyts, BCCs, and palmar/plantar pits, and the alteration was reportedly found to segregate with disease in the family (Sun, LS et al. J Dent Res. 2008 Jun;87(6):575-9). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.47% (greater than 200 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
