This sequence change replaces serine, which is neutral and polar, with leucine, which is neutral and non-polar, at codon 419 of the FH protein (p.Ser419Leu). This variant has not been reported in the literature in individuals affected with FH-related conditions. ClinVar contains an entry for this variant (Variation ID: 429183). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FH protein function. This variant disrupts the p.Ser419 amino acid residue in FH. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 15937070, 16597677; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The p.S419L variant (also known as c.1256C>T), located in coding exon 9 of the FH gene, results from a C to T substitution at nucleotide position 1256. The serine at codon 419 is replaced by leucine, an amino acid with dissimilar properties. This variant has been observed in multiple individuals with features consistent with hereditary leiomyomatosis and renal cell cancer (Feng D et al. Transl Androl Urol, 2020 Apr;9:789-793; Äyräväinen A et al. Hum Reprod, 2020 Oct;35:2237-2244; Sarkadi B et al. Cancers (Basel), 2021 Aug;13; Yin X et al. Virchows Arch, 2024 Apr; Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this variant is interpreted as a disease-causing mutation. -
