Variant summary

Our verdict is Likely pathogenic. Variant got 8 ACMG points: 8P and 0B. PM2PP3_StrongPP5_Moderate

The NM_000061.3(BTK):c.-31+5G>T variant causes a splice donor 5th base, intron change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predicting alterations to normal splicing. Variant has been reported in ClinVar as Likely pathogenic (★).

Genes affected
BTK (HGNC:1133): (Bruton tyrosine kinase) The protein encoded by this gene plays a crucial role in B-cell development. Mutations in this gene cause X-linked agammaglobulinemia type 1, which is an immunodeficiency characterized by the failure to produce mature B lymphocytes, and associated with a failure of Ig heavy chain rearrangement. Alternative splicing results in multiple transcript variants encoding different isoforms. [provided by RefSeq, Dec 2013]

Likely pathogenic, criteria provided, single submitterclinical testingGeneDxMay 05, 2017The c.-31+5 G>T splice site variant in the BTK gene has been previously reported in association with X-linked agammaglobulinemia (Shin et al., 2008). In silico splice prediction models predict that c.-31+5 G>T damages the natural splice donor site for intron 1; however, the adjacent exon 1 is non-coding. Functional studies have shown that the c.-31+5 G>T variant reduces transcriptional activity and the expression of the BTK protein (Shin et al., 2008). In summary, we consider this variant to be likely pathogenic. -
