Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_172107.4(KCNQ2):c.204delC(p.Lys69SerfsTer64) variant causes a frameshift change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.000000721 in 1,386,538 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. P68P) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Developmental and epileptic encephalopathy Pathogenic:1
This sequence change creates a premature translational stop signal (p.Lys69Serfs*64) in the KCNQ2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in KCNQ2 are known to be pathogenic (PMID: 14534157, 23692823, 27779742). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with KCNQ2-related conditions. ClinVar contains an entry for this variant (Variation ID: 817818). For these reasons, this variant has been classified as Pathogenic. -
not provided Pathogenic:1
The c.204delC pathogenic variant in the KCNQ2 gene causes a frameshift starting with codon Lysine 69, changes this amino acid to a Serine residue and creates a premature Stop codon at position 64 of the new reading frame, denoted p.Lys69SerfsX64. This pathogenic variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The c.204delC variant is not observed in large population cohorts (Lek et al., 2016). Although this pathogenic variant has not been previously reported to our knowledge, its presence is consistent with the diagnosis of a KCNQ2-related disorder in this individual. -
