Variant summary: SPTAN1 c.2572G>T (p.Ala858Ser) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251122 control chromosomes. c.2572G>T has been reported in the literature in the heterozygous state in at least 2 individuals affected with clinical features of SPTAN1-related hereditary spastic paraplegia (example, Leveille_2019). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 31515523). ClinVar contains an entry for this variant (Variation ID: 530408). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic.

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The serine amino acid residue is found in multiple mammalian species, suggesting that this missense change does not adversely affect protein function. These predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This sequence change replaces alanine with serine at codon 858 of the SPTAN1 protein (p.Ala858Ser). The alanine residue is highly conserved and there is a moderate physicochemical difference between alanine and serine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with SPTAN1-related disease.
