Variant summary
Our verdict is Uncertain significance. Variant got 4 ACMG points: 4P and 0B. PM2_SupportingPP3PM1
This summary comes from the ClinGen Evidence Repository: The NM_000018.4: c.640T>G (p.Phe214Val) in ACADVL is a missense variant predicted to cause substitution of phenylalanine by valine at amino acid 214 (p.Phe214Val). The highest population minor allele frequency in gnomAD v2.1.1 is 0.000009 in Non-Finnish European population, which is lower than the ClinGen ACADVL Variant Curation Expert Panel threshold (<0.001) for PM2_Supporting, meeting this criterion (PM2_Supporting). This variant has been reported in the literature in five individuals with an abnormal newborn screen who are apparently heterozygous carriers for the variant (PMID:26385305), but this information is insufficient to use toward classification. This variant resides within a region, amino acids 214–223, of ACADVL that is defined as a mutational hotspot and critical functional domain by the ClinGen ACADVL Variant Curation Expert Panel (PMIDs: 20060901, 9973285; PM1). The computational predictor REVEL gives a score of 0.916, which is above the threshold of 0.75, evidence that correlates with impact to ACADVL function (PP3). Due to limited evidence, this variant is classified as a variant of uncertain significance for autosomal recessive very long chain acyl-CoA dehydrogenase (VLCAD) deficiency based on the ACMG/AMP criteria applied, as specified by the ClinGen ACADVL Variant Curation Expert Panel: PM1, PM2_Supporting, PP3 (ACADVL VCEP specifications version 1; approved November 9, 2021). Likely pathogenic, criteria provided, single submitter | clinical testing | Wong Mito Lab, Molecular and Human Genetics, Baylor College of Medicine | Nov 01, 2019 | The NM_000018.3:c.640T>G (NP_000009.1:p.Phe214Val) [GRCH38: NC_000017.11:g.7221969T>G] variant in ACADVL gene is interpretated to be Likely Pathogenic based on ACMG guidelines (PMID: 25741868). This variant has been reported. This variant meets the following evidence codes reported in the ACMG guidelines: PS3, PM1, PM2, PP3 - |
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Apr 28, 2023 | This missense change has been observed in individual(s) with abnormal newborn screening results suggestive of very long chain acyl-coA dehydrogenase deficiency (PMID: 26385305). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ACADVL protein function. ClinVar contains an entry for this variant (Variation ID: 541723). This variant is present in population databases (no rsID available, gnomAD 0.0009%). This sequence change replaces phenylalanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 214 of the ACADVL protein (p.Phe214Val). - |

Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | Sep 25, 2020 | Observed as heterozygous in 5 individuals identified by newborn screening; detailed clinical information and information about the presence of a second variant in ACADVL was not reported (Miller et al., 2015); Not observed at a significant frequency in large population cohorts (Lek et al., 2016); In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect; This variant is associated with the following publications: (PMID: 26385305) - |

Uncertain significance, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Jun 17, 2024 | Variant summary: ACADVL c.640T>G (p.Phe214Val) results in a non-conservative amino acid change located in the Acyl-CoA oxidase/dehydrogenase, middle domain (IPR006091) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251346 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.640T>G has been reported in the literature in individuals who were identified by newborn screening with Very Long Chain Acyl-CoA Dehydrogenase Deficiency (Miller_2015). However, the zygosity of these individuals is unknown. These report(s) do not provide unequivocal conclusions about association of the variant with Very Long Chain Acyl-CoA Dehydrogenase Deficiency. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 26385305). ClinVar contains an entry for this variant (Variation ID: 541723). Based on the evidence outlined above, the variant was classified as uncertain significance. - |
