Variant summary
Our verdict is Pathogenic. Variant got 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong
The NM_000398.7(CYB5R3):c.250C>T(p.Arg84*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000889 in 1,461,668 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Variant results in nonsense mediated mRNA decay.

Pathogenic, criteria provided, single submitter | clinical testing | GeneDx | May 31, 2017 | The R84X variant in the CYB5R3 gene has been reported previously in the homozygous state, in an individual with type II hereditary methemoglobinaemia, characterized by preterm birth, developmental delay, absent speech, stiff and flexed limbs, and marked decrease in b5 reductase activity in both erythrocytes and leukocytes; unaffected parents and one sibling of this individual were all heterozygous for the R84X variant (Higasa et al., 1998). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The R84X variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). We interpret R84X as a pathogenic variant. - |
