Variant summary: PTS c.26G>A (p.Arg9His) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 2.5e-06 in 1581080 control chromosomes (gnomAD v4.1). c.26G>A has been observed in multiple individuals affected with 6-Pyruvoyl-Tetrahydropterin Synthase Deficiency (e.g. Ye_2013, Kuznetcova_2019, Gundorova_2021). These data indicate that the variant is very likely to be associated with disease. The following publications have been ascertained in the context of this evaluation (PMID: 22237589, 23138986, 31332730, 33822819). ClinVar contains an entry for this variant (Variation ID: 552463). Based on the evidence outlined above, the variant was classified as pathogenic. -

Other variant(s) that disrupt this residue have been observed in individuals with PTS-related conditions (PMID: 20059486), which suggests that this may be a clinically significant amino acid residue. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Tolerated"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The histidine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. ClinVar contains an entry for this variant (Variation ID: 552463). This missense change has been observed in individual(s) with tetrahydrobiopterin deficiency (PMID: 22237589, 23138986, 31332730, 33822819). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 9 of the PTS protein (p.Arg9His). -
