CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. The c.1373G>A variant is not present in publicly available population databases like 1000 Genomes, EVS, gnomAD, ExAC, Indian Exome Database or our in-house exome database. This variant has been previously observed in a female child affected with CFTR-related conditions [PMID: 36038301]. It has not been reported to the clinical databases like ClinVar, Human Genome Mutation Database (HGMD) or OMIM. In silico pathogenicity prediction programs like SIFT, PolyPhen-2, MutationTaster2, CADD, Varsome, Franklin, InterVar etc predicted this variant to be likely deleterious. This variant is located in a mutational hotspot region of the gene and a different amino acid change in the same codon (c.1373G>T, p.Gly458Val) has been previously observed in the affected individuals, published in literature and reported to the databases as ‘pathogenic / likely pathogenic’ by multiple submitters. This variant has been identified in a family with an affected proband in homozygous state, parents are carriers. -
