The arginine at codon 898 is replaced by glutamine, an amino acid with highly similar properties. This alteration was identified in multiple family members diagnosed with idiopathic epilepsy and reportedly co-segregated with disease (Kapoor A et al. Ann Neurol, 2008 Aug;64:158-67). Additionally, in vitro studies showed that R898Q enhances targeting to the plasma membrane and increases extracellular Ca2+-stimulated ERK1/2 phosphorylation (Stepanchick A et al. Cell Physiol Biochem, 2010 Aug;26:363-74). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. In addition, the evidence for the gene-disease relationship is limited for pancreatitis and cancer predisposition; therefore, the clinical significance of this variant for CASR-related pancreatitis and cancer predisposition is unclear.
