The acetylcholine receptor changes subunit composition shortly after birth when the epsilon subunit replaces the gamma subunit seen in embryonic receptors. This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 141 of the CHRNE protein (p.Pro141Leu). This variant is present in population databases (rs121909512, gnomAD 0.003%). This missense change has been observed in individuals with autosomal recessive congenital myasthenic syndrome (PMID: 8755487, 17878953; Invitae). It has also been observed to segregate with disease in related individuals. This variant is also known as P121L. ClinVar contains an entry for this variant (Variation ID: 18346). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CHRNE protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects CHRNE function (PMID: 8755487). For these reasons, this variant has been classified as Pathogenic. -
