This sequence change replaces tryptophan, which is neutral and slightly polar, with cysteine, which is neutral and slightly polar, at codon 627 of the GUSB protein (p.Trp627Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with mucopolysaccharidosis type VII (PMID: 7680524). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 907). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GUSB protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change affects GUSB function (PMID: 7680524). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The GUSB c.1881G>T variant is predicted to result in the amino acid substitution p.Trp627Cys. This variant was reported in the compound heterozygous state in an individual with mucopolysaccharidosis VII (Shipley et al. 1993. PubMed ID: 7680524; Vervoort et al. 1996. PubMed ID: 8644704). Functional analysis of this variant showed a reduction in enzyme activity to ~ 40-65% of wild type activity (Shipley et al. 1993. PubMed ID: 7680524; Tomatsu et al. 2009. PubMed ID: 19224584). This variant has not been reported in a large population database (http://gnomad.broadinstitute.org), indicating this variant is rare. In ClinVar, this variant has been interpreted as likely pathogenic and pathogenic (https://www.ncbi.nlm.nih.gov/clinvar/variation/907). Based on this evidence, we interpret this variant as likely pathogenic. -
