The encoded protein is primarily composed of 22 spectrin repeats which are involved in dimer formation. It forms a component of the erythrocyte plasma membrane. In vitro functional analyses demonstrate loss of spectrin tetramerization (Gaetani 2008). This variant is also reported in ClinVar (Variation ID: 12853). This variant is rarely found in the African/African-American population with an allele frequency of 0.008% (2/24,196 alleles) in the Genome Aggregation Database, indicating it is not a common polymorphism. Additionally, other amino acid substitutions at this codon (Cys, Ser and His) have been reported in individuals with hereditary elliptocytosis and pyropoikilocytosis and are considered pathogenic (Coetzer 1991, Floyd 1991, Garbarz 1990). The arginine at codon 28 is highly conserved, but computational analyses are uncertain whether variant is neutral or deleterious (REVEL: 0.44). This missense change has been observed in individual(s) with elliptocytosis and hereditary pyropoikilocytosis (PMID: 1679439, 3940543). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 12853). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SPTA1 protein function. This variant disrupts the p.Arg28 amino acid residue in SPTA1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 18218854, 29729090, 30317022). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
