In vitro functional analyses demonstrate loss of spectrin tetramerization (Gaetani 2008). This variant is also reported in ClinVar (Variation ID: 12853). This variant is rarely found in the African/African-American population with an allele frequency of 0.008% (2/24,196 alleles) in the Genome Aggregation Database, indicating it is not a common polymorphism. Additionally, other amino acid substitutions at this codon (Cys, Ser and His) have been reported in individuals with hereditary elliptocytosis and pyropoikilocytosis and are considered pathogenic (Coetzer 1991, Floyd 1991, Garbarz 1990). The arginine at codon 28 is highly conserved, but computational analyses are uncertain whether variant is neutral or deleterious (REVEL: 0.44). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SPTA1 protein function.
