Genes affected
SCN1A (HGNC:10585): (sodium voltage-gated channel alpha subunit 1) Voltage-dependent sodium channels are heteromeric complexes that regulate sodium exchange between intracellular and extracellular spaces and are essential for the generation and propagation of action potentials in muscle cells and neurons. Each sodium channel is composed of a large pore-forming, glycosylated alpha subunit and two smaller beta subunits. This gene encodes a sodium channel alpha subunit, which has four homologous domains, each of which contains six transmembrane regions. Allelic variants of this gene are associated with generalized epilepsy with febrile seizures and epileptic encephalopathy. Alternative splicing results in multiple transcript variants. The RefSeq Project has decided to create four representative RefSeq records. Three of the transcript variants are supported by experimental evidence and the fourth contains alternate 5' untranslated exons, the exact combination of which have not been experimentally confirmed for the full-length transcript. [provided by RefSeq, Oct 2015]
SCN1A-AS1 (HGNC:54069): (SCN1A and SCN9A antisense RNA 1)

Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpOct 24, 2022For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Gly1433 amino acid residue in SCN1A. Other variant(s) that disrupt this residue have been observed in individuals with SCN1A-related conditions (PMID: 20729507, 21248271), which suggests that this may be a clinically significant amino acid residue. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SCN1A protein function. ClinVar contains an entry for this variant (Variation ID: 68630). This missense change has been observed in individual(s) with Dravet syndrome (PMID: 18554359, 23762420). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces glycine, which is neutral and non-polar, with glutamic acid, which is acidic and polar, at codon 1433 of the SCN1A protein (p.Gly1433Glu). -

Pathogenic, criteria provided, single submitterclinical testingGeneDxJun 22, 2017An G1433E published pathogenic variant has been identified in the SCN1A gene. The G1433E variant has been reported previously as a de novo pathogenic variant in patients with a clinical diagnosis of Dravet syndrome (Verbeek et al., 2013; Sun et al., 2008; SCN1A Variant Database). Additionally, a different missense substitution at the same codon (G1433R) as well as multiple missense variants in nearby residues have been reported in the Human Gene Mutation Database in association with SCN1A-related disorders ((Nicita et al., 2010; Stenson et al., 2014), supporting the functional importance of this region of the protein. G1433E alters a highly conserved position predicted to be within the pore forming loop between the S5 and S6 transmembrane segments of the third homologous domain,. The G1433E variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). Furthermore, the G1433E variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties and in silico analysis predicts this variant is probably damaging to the protein structure/function. Therefore, the presence of G1433E is consistent with the diagnosis of an SCN1A-related disorder in this individual. -
