When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. This variant was reported in an individual who met Amsterdam I/II criteria for Lynch syndrome and tumor demonstrated high microsatellite instability (Pistorius SR et al. Int J Colorectal Dis. 2000 Nov;15(5-6):255-63). This variant has been identified in a proband whose Lynch syndrome-associated tumor demonstrated loss of MSH2 expression by immunohistochemistry (Ambry internal data). This variant has also been identified as somatic in conjunction with a somatic pathogenic MSH2 variant in a tumor that demonstrated high microsatellite instability and loss of MSH2/MSH6 expression by immunohistochemistry (Haraldsdottir S et al. Gastroenterology. 2014 Dec;147(6):1308-1316.e1; Ambry internal data). RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -
