ABCD1 (HGNC:61): (ATP binding cassette subfamily D member 1) The protein encoded by this gene is a member of the superfamily of ATP-binding cassette (ABC) transporters. ABC proteins transport various molecules across extra- and intra-cellular membranes. ABC genes are divided into seven distinct subfamilies (ABC1, MDR/TAP, MRP, ALD, OABP, GCN20, White). This protein is a member of the ALD subfamily, which is involved in peroxisomal import of fatty acids and/or fatty acyl-CoAs in the organelle. All known peroxisomal ABC transporters are half transporters which require a partner half transporter molecule to form a functional homodimeric or heterodimeric transporter. This peroxisomal membrane protein is likely involved in the peroxisomal transport or catabolism of very long chain fatty acids. Defects in this gene have been identified as the underlying cause of adrenoleukodystrophy, an X-chromosome recessively inherited demyelinating disorder of the nervous system. Variant summary: ABCD1 c.443A>G (p.Asn148Ser) results in a conservative amino acid change located in the ABC transporter type 1, transmembrane domain (IPR011527) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 180453 control chromosomes. c.443A>G has been reported in the literature in the hemizygous and heterozygous states in multiple individuals affected with Addison disease and/or cerebral adrenoleukodystrophy (child and adult onset) (example, Coll_2005, Feigenbaum_1996, Ganapathy_2019, Matsukawa_2011, Salsano_2012, Takano_1999). These data indicate that the variant is very likely to be associated with disease. The following publications have been ascertained in the context of this evaluation (PMID: 15811009, 8651290, 31069529, 20661612, 22120190, 10190819). This missense change has been observed in individual(s) with X-linked adrenoleukodystrophy (PMID: 7849723, 29390592, 31069529). ClinVar contains an entry for this variant (Variation ID: 11297). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ABCD1 protein function with a positive predictive value of 95%.
