The p.Q228* pathogenic mutation (also known as c.682C>T), located in coding exon 4 of the PALB2 gene, results from a C to T substitution at nucleotide position 682. This changes the amino acid from a glutamine to a stop codon within coding exon 4. This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

Variant summary: PALB2 c.682C>T (p.Gln228X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant was absent in 251096 control chromosomes. c.682C>T has been reported in the literature in at least one individual affected with breast cancer (e.g. Yang_2020). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Four submitters, including one expert panel (ClinGen), have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
