Variant summary
Our verdict is Uncertain significance. The variant received 3 ACMG points: 3P and 0B. PM2PM4_Supporting
The NM_004656.4(BAP1):c.670_672delCAC(p.His224del) variant causes a conservative inframe deletion change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,461,462 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. H224H) has been classified as Likely benign.

BAP1-related tumor predisposition syndrome Uncertain:1
ClinVar contains an entry for this variant (Variation ID: 539905). This variant, c.670_672del, results in the deletion of 1 amino acid(s) of the BAP1 protein (p.His224del), but otherwise preserves the integrity of the reading frame. This variant is present in population databases (no rsID available, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with BAP1-related conditions. Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The c.670_672delCAC variant (also known as p.H224del) is located in coding exon 9 of the BAP1 gene. This variant results from an in-frame CAC deletion at nucleotide positions 670 to 672. This results in the in-frame deletion of a histidine at codon 224. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
