This sequence change replaces glutamine, which is neutral and polar, with arginine, which is basic and polar, at codon 379 of the POLD1 protein (p.Gln379Arg). RNA analysis indicates that this missense change induces altered splicing and may result in an absent or disrupted protein product. However, the current clinical and genetic evidence is not sufficient to establish whether loss-of-function variants in POLD1 cause disease. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with POLD1-related conditions. ClinVar contains an entry for this variant (Variation ID: 469177). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. Studies have shown that this missense change results in skipping of exon 9 and introduces a premature termination codon (internal data). The resulting mRNA is expected to undergo nonsense-mediated decay. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. The p.Q379R variant (also known as c.1136A>G), located in coding exon 8 of the POLD1 gene, results from an A to G substitution at nucleotide position 1136. The glutamine at codon 379 is replaced by arginine, an amino acid with highly similar properties. This amino acid position is well conserved through mammals but not in all available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
