MYH9 (HGNC:7579): (myosin heavy chain 9) This gene encodes a conventional non-muscle myosin; this protein should not be confused with the unconventional myosin-9a or 9b (MYO9A or MYO9B). The encoded protein is a myosin IIA heavy chain that contains an IQ domain and a myosin head-like domain which is involved in several important functions, including cytokinesis, cell motility and maintenance of cell shape. Defects in this gene have been associated with non-syndromic sensorineural deafness autosomal dominant type 17, Epstein syndrome, Alport syndrome with macrothrombocytopenia, Sebastian syndrome, Fechtner syndrome and macrothrombocytopenia with progressive sensorineural deafness. Variant summary: MYH9 c.5026A>G (p.Lys1676Glu) results in a conservative amino acid change located in the myosin tail domain (IPR002928) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 0.00033 in 250738 control chromosomes (gnomAD). To our knowledge, no occurrence of c.5026A>G in individuals affected with Macrothrombocytopenia And Granulocyte Inclusions With Or Without Nephritis Or Sensorineural Hearing Loss and no experimental evidence demonstrating its impact on protein function have been reported. Six submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as benign/likely benign. Based on the evidence outlined above, the variant was classified as uncertain significance. -

The Lys1676Glu variant in MYH9 has not been reported in individuals affected wit h hearing loss, but has been identified in 0.1% (8/8600) of European American ch romosomes in a broad population by the NHLBI Exome sequencing project (http://ev s.gs.washington.edu/EVS/; dbSNP rs138158369). Although this variant has been see n in the general population, a pathogenic role cannot be ruled out due to variab le expressivity and onset of hearing loss associated with MYH9 variants (Pecci 2 008). Computational analyses (biochemical amino acid properties, conservation, A lignGVGD, PolyPhen2, and SIFT) do not provide strong support for or against an i mpact to the protein. In summary, additional data is needed to determine the cli nical significance of this variant. -
